This program supports older adults in accessing social and community resources that strengthen their physical, emotional, and social well-being.
Through friendly one-on-one conversations, Community Connectors work with participants to explore what matters most to them, develop a personalized wellness plan, and connect them with meaningful activities, programs, and supports. The program takes a holistic, strengths-based approach that recognizes each person’s experiences, goals, and abilities.
Community Connectors can provide:

One-on-one, supportive conversations

Connections to social programs, recreation opportunities, and volunteering

Assistance with forms, transportation resources, housing information, and navigating community services

Interpretation support, available upon request
Please note that Community Connectors do not provide medical care, counselling, financial assistance, or clinical treatment.
Participants may benefit if they are:

Feeling lonely or socially isolated

Less active or experiencing mobility challenges

Living with limited income that affects basic needs such as food or medications

Living without family or other support nearby

Experiencing housing concerns or unstable living conditions

Going through a major life change, such as the loss of a spouse

Visiting doctors or emergency services frequently due to social factors
Referrals are welcome from:
