Community Connector

This program supports older adults in accessing social and community resources that strengthen their physical, emotional, and social well-being.

Through friendly one-on-one conversations, Community Connectors work with participants to explore what matters most to them, develop a personalized wellness plan, and connect them with meaningful activities, programs, and supports. The program takes a holistic, strengths-based approach that recognizes each person’s experiences, goals, and abilities.

Community Connectors can provide:

  • One-on-one, supportive conversations

  • Connections to social programs, recreation opportunities, and volunteering

  • Assistance with forms, transportation resources, housing information, and navigating community services
